- Judo Manitoba – Assumption of Risk 2026–2027 [PDF] – to be completed by parents/guardians of members under the age of 18
- Judo Manitoba – Waiver 2026–2027 [PDF] – to be completed by members 18 years and older
Note:
- The registration form needs to be completed by everyone.
- The Assumption of Risk form is to be completed by parents/guardians of registrants under 18.
- The Waiver is to be completed by registrants 18 and over.
